A complete physical exam is mandated for students in grades K, 4 and 8.
These exams should be provided by the student's primary care physician.
All transfer students entering the Topsfield Public Schools must have written proof of a physical examination, performed by their private physician, within one year of the student's date of entry.
| Massachusetts now offers free or low cost health insurance to all children and teens. Please contact your school nurse for more information. All communication will remain confidential. |
REQUIRED IMMUNIZATIONS
All immunizations must meet Massachusetts Department of Public Health
requirements and must be certified in writing by a licensed physician. State law
requires the following immunizations:

| THESE ENTRY REQUIREMENTS MUST BE MET BEFORE THE STUDENT IS ALLOWED TO START SCHOOL. YOUR CHILD WILL BE EXCLUDED FROM SCHOOL UNTIL THE REQUIREMENTS ARE MET. |
