Immunization Requirements
Below are the State mandated requirements
for all incoming students:
DTP 5 doses
Polio 4 doses
Hepatitis B 3 doses
MMR 2 doses
Varicella 1 doses
Other Mandated Requirements
A copy of the 5- year physical
The result of a lead test done after
3 years of age.
